Diagnostic Automation 17-Hydroxyprogesterone (17-OH progesterone or 17OHP) is a C-21 steroid hormone
produced in the adrenal gland and gonads, during the synthesis of glucocorticoids and sex steroids. It is derived
from progesterone via 17-hydroxylase, a P450c17 enzyme, or from 17-hydroxypregnenolone via 3β-hydroxysteroid
dehydrogenase/Δ5-4 isomerase.
17α -OHP has no defined physiologic role except as a precursor molecule.
Serum 17α -OHP levels are age-dependent, with peak levels observed during fetal life and the immediate postnatal

Congenital Adrenal Hyperplasia (CAH) includes a group of disorders, in which the cortisol production by
adrenals is lowered. There are several different forms of CAH, each related to one of the enzymes
necessary to transform precursors to cortisol. When one of these enzymes is deficient, this leads to a
hyper function and increased size (hyperplasia) of the adrenals.
Among the various forms of CAH, the 21-hydroxylase deficiency is by far the most frequent, representing

The steroid 17-α-Hydroxyprogesterone (17-α-OHP) is produced by both the adrenal cortex and gonads. Even though 17-α-OHP has relatively little progestational activity, it is of intense clinical interest because it is the immediate precursor to 11 -desoxycortisol (Cpd-S). Because Cpd-S is produced by 21 -hydroxylation of 1 7-α-OHP, measurement of 1 7-α-OHP is a useful indirect indicator of 21 -hydroxylase activity.
